1. Please complete the form below providing us with your NAME and EMAIL. You will receive a PAYMENT CODE that you will need to register on the KIDS SAFE website.
2. After you receive your code, please PRINT or DOWNLOAD the INSTRUCTIONS for the KIDS SAFE website.
3. Go to the KIDS SAFE website and follow the instructions provided using your PAYMENT CODE.
